Basically, people have 2 options: either go public or pay a monthly fee to get medical coverage.
The public system is paid for by the authorities and overcrowded. Besides all other problems that can affect its quality, this is one of the causes that makes it even worst.
So, brazilians that can afford it, are left with private systems that are mainly health maintenance organizations (HMO) and insurances that reimburse medical expenses.
People that go for an HMO, do not have to advance anything, but on the other hand, are not able to choose the doctor and/or hospital: these organizations have their providers.
Whenever you decide to buy an insurance, you will be reimbursed for medical visits and will be taken in charge without any advancement for lab workups and hospitalisations. In this case, medical fees are limited to a certain amount (it depends on how much you pay monthly) and laboratory and hospital options may vary once more depending on the monthly fare
